We’re Hiring: Community Engagement Worker

· Total weekly hours: 37 hours per week

NOTE: This is a 1-year fixed contract, with the view to extended dependant on funding.
Main Purpose of Role
To engage and facilitate new referrals to our organisation, offering face-to-face advice and guidance in the Holbeck community. We aim to encourage positive, healthy behaviours and lifestyle changes, thereby improving overall health outcomes within the community. By fostering a supportive environment and providing essential resources, we strive to enhance the well-being of all residents in Holbeck.
Duties & Key Responsibilities
Main Duties
The community-based role is ideal for a highly motivated individual committed to supporting people and becoming an integral part of the community. The main duties include:
· One-to-One Assessments: Conduct personalised assessments to understand individual needs and circumstances.
· Mapping of the LS11 Ward: Systematically identify and document resources, needs, and opportunities within the LS11 ward.
· Group Setup: Establish groups that align with the organization’s core objectives, vision, and purpose.
· Holistic Social Activities Program: Develop and offer a comprehensive program of social activities that incorporate health and well-being information, advice, and intensive individual support to encourage engagement with Holbeck Together services and other organizations.
· Multi-Agency Collaboration: Work with multi-agency partners to provide a person-centred approach.
· Signposting and Referrals: Direct individuals to relevant services and resources.
· Equal and Effective Service Delivery: Ensure services are delivered equitably and effectively to all community members.
· Promotion of Initiatives and Strategies: Advocate for local and citywide initiatives and strategies that benefit the community.
· Referral Mechanisms: Collaborate with local GP practices and partners to establish referral mechanisms for their patients and clients.
· Positive and Supportive Atmosphere: Foster a welcoming and encouraging environment.
· Organisational Promotion: Promote the organisation and its accomplishments both internally and externally.
· External Partnerships: Build and manage partnerships that lead to a sustainable client base.
· Monitoring and Evaluation: Complete a structured Monitoring and Evaluation process to ensure that the services provided are effectively evaluated, and the data collected is meaningful and actionable. Regularly updating the process based on feedback and findings helping Holbeck Together continuously improving the service and demonstrating its impact to stakeholders.
